What is Interventional Physiotherapy?
Interventional Physiotherapy is a physiotherapy approach that actively uses targeted hands-on techniques and therapeutic interventions to reduce pain, restore movement and accelerate recovery, rather than relying primarily on exercise alone.
Is this your experience with your last physio?
You walk in and out of a treatment room feeling exactly the same.
You tell them about your problem, they spend 20 minutes asking questions and assessing your condition.
They tell you to do a list of exercises 15 reps 3 sets and 3 times a day.
Before they send you off, they throw in a 2 minutes massage, and if you are lucky maybe 2 acupuncture needles that are not even where the pain is.
They tell you exercise is the gold standard for LONG TERM RECOVERY.
And then you leave with the same amount of pain you come in with.
There is a gap in pain care
What regular physio cares: research says patient need to do exercises to make long term improvement. So, my job is to give exercises to patients.
What patient thinks: I come in to see a physio because I have pain now and I want it gone. I want to be healthy long term, but I need to you to help me so I can sleep/work/play/rest without pain.
Cost of regular physio:
Research has consistently demonstrated effectiveness of exercises supervised by a qualified personnel for painful conditions. It usually involves 2x one-hour sessions per week over the course of 12 weeks.
It translates to 24 supervised hours in 3 months.
Or patient can sleep/work/play/rest without pain only after 3 months.
Time cost: Patient wants pain gone now not in 3 months
Financial cost: 24 hour of supervised exercise by a physiotherapist means over $6000 invested in a simple lower back pain. Alternatively, most physio will actually ask you to do exercises unsupervised at home, which challenges your commitment.
Commitment cost: 2 hour per week exercising plus commute time to and from the physio, or close to 35 hours in 3 months. Alternatively, if you do it at home, patient’s commitment are often challenged because they don’t see immediate improvement yet the exercises takes them away from work that they see immediate urgency and outcome e.g. have a fed the cat, cooking diner for the family, project deadline.
Set for failure
With the abovementioned costs, patient will naturally compare physio with other alternatives:
·Pain medication: Patient feel pain relief, if not a cure immediately. They can buy Panadol that last them for 20 years for they same price the pay the physio. It only takes 2 second to swallow a tablet
·Cortisone injection: Patient can feel relief with a once off cost of $200. It takes 15 minutes.
·Surgery: It will take time and some pain during recovery. If they have insurance, it cost $800 in excess and $2000 in insurance premium a year, still cheaper than the $6000 they spent with the physio.
It appears that regular physio is costly, time consuming and require significant commitment to succeed. Frankly, it is apparent that patient would not even think to see a regular physio.
What Does Interventional Physiotherapy Mean to our Patients?
If you're in pain, unable to move comfortably, or struggling to perform everyday activities, simply being given exercises may not be enough. Pain needs to be reduced
Interventional Physios emphasise on the meaningful outcomes brought to patient by the right treatments and the right time.
Different conditions respond to different interventions:
A persistent tendon injury may benefit from Shockwave Therapy to help stimulate recovery, but different parameters and settings a early stage of recovery compare to mid to late stage
Tight, painful muscles may respond to Dry Needling or Manual Therapy to reduce discomfort and improve movement, but appropriate needling placement and manual depth with acute versus chronic presentations.
Joint stiffness may improve with targeted mobilisation techniques. Posterioanterior glide versus traction when you have a whipash injury 2 days ago vs 3 years ago.
Acute or persistent pain may require a combination of interventions before progressing into strengthening and exercise.
It means interventional physiotherapists choose the right treatments that they know will help you sleep/work/play/rest the moment after your appointment. You would know the differences the moment you walk out of the rooms.

Sarah had been experiencing persistent shoulder pain for several months. She had seen a physio and tried exercises and stretching but continued to struggle with daily activities and sleep.
The problem
While exercise is an important part of shoulder recovery, Sarah's pain was limiting her ability to participate effectively in rehabilitation.

Strengthening exercises are important for tendon recovery. However, John's symptoms were preventing him from loading the tendon effectively.

Dry needling directly over the fusiform of the achilles tendon and calves muscles and plantar fascia that contributes to him pain
